Rehab Referral Agreement

Your pet has been referred to the Rehabilitation and Fitness Department at Care Pets Animal Hospital and Wellness Center for evaluation and/or diagnostics and treatment. The veterinarian that referred you to us has confidence in our advanced training to care for your pet’s current medical or rehabilitation needs. We wish to address your pet’s medical condition to the best of our ability and then have you return to your veterinarian for your pet’s routine care. We request that you keep your pet up to date on vaccinations and provide us with a copy of your pet’s current record for our files.

In this spirit of referral, please sign below. By signing, you are agreeing to not ask us to care for any other problems your pet may have other than the one for which you were referred, and to see your regular veterinarian for any issues that do not pertain to the referred problem or pet.
